A comparative study of electroacupuncture at Zhongliao (BL33) and other acupoints for overactive bladder

摘要:

Electroacupuncture (EA) at Zhongliao (BL33) can improve the symptoms of overactive bladder (OAB), such as urinary frequency, urgency, and incontinence. However, its performance compared with other acupoints remains unclear. This study investigated the effects of EA at BL33 with deep needling on rats with OAB by detecting urodynamics in eight groups: no intervention group, D-BL33 group (deep needling at BL33), S-BL33 group (shallow needling at BL33), non-acupoint group (needling at the non-acupoint next to BL33), Weizhong (BL40) group, Sanyinjiao (SP6) group, Tongtian (BL7) group, and Hegu (LI4) group. Results revealed that EA at BL33 with deep needling, BL40, and SP6 prolonged the intercontraction interval (ICI) of rats with OAB (P=0.001, P=0.005, P=0.046, respectively, post-treatment vs. post-modeling). Furthermore, the change in ICI from post-modeling in the D-BL33 group was significantly greater than those of the no intervention and other EA groups (all P<0.01). Significantly shortened vesical micturition time (VMT) and elevated maximum detrusor pressure (MDP) were also observed in the D-BL33 group (P=0.017 and P=0.024, respectively, post-treatment vs. post-modeling). However, no statistically significant differences in the changes of VMT and MDP from post-modeling were observed between D-BL33 and the other EA groups. In conclusion, EA at BL33 with deep needling may inhibit acetic-acid-induced OAB more effectively.

Dynamics analysis of bladder-urethra system based on CFD

摘要: A mathematical model for a bladder-urethra system can provide basic analysis for the disabled urethra closure of stress urinary incontinence (SUI) patients in a clinic. Based on computational fluid dynamics (CFD), we developed a three-dimensional urodynamic bladder-urethra system, which includes bladder, bladder neck, prostate, and urethra. The realistic recirculation process of the urinary bladder during the physiologic voiding process in conjunction with a flow simulation through the female urinary bladder and urethra is presented. The computational results show that a dead-water zone and the zone of secondary flow occur, independent of the shape of the prostatic urethra. For the pathological prostata, the extreme constriction of the prostatic urethra results in an additional wide-stretched dead-water zone. The simulation results does not only improve urinary incontinence surgery for clinicians, but can also provide a basis for theoretical analysis.

Small cell carcinoma of the urinary bladder without gross hematuria: a case report

摘要:

Small cell carcinoma of the urinary bladder (SCCB) is a rare and aggressive form of bladder cancer with poor prognosis. Hematuria is the main symptom of this malignancy, and most patients have a history of smoking. The disease incidence of malignant bladder tumors in China is approximately 0.74%. Early and accurate diagnosis of SCCB can ensure timely and appropriate treatment of this malignant disease. Oncologic surgery is the standard treatment; however, it may not be a curative approach. Chemotherapy and/or radiotherapy should be performed following surgical removal. This case report describes a patient with a single neoplasm diagnosed as SCCB that arose because of recurrence of bladder cancer after bladder tumor resection. In contrast to previously reported cases, this patient had no gross hematuria and no history of smoking.

Can intravesical bacillus Calmette-Guérin reduce recurrence in patients with non-muscle invasive bladder

Approximately 70% of newly diagnosed bladder tumors are non-muscle invasive bladder cancer (NMIBC). NMIBC accounts for approximately 80% of total bladder cancer cases. Bacillus Calmette-Guérin (BCG) instillation and maintenance is considered as the standard adjuvant treatment for superficial bladder cancer. A number of randomized studies have focused on the benefit of maintenance therapy following initial BCG induction. To provide further insights into the effect of intravesical instillation on recurrence in patients with NMIBC, we analyzed this relationship by conducting an updated detailed meta-analysis. Evidence suggested that adjuvant intravesical BCG with maintenance treatment is significantly effective for the prophylaxis of tumor recurrence in patients with NMIBC.

摘要: Meta-analysis was used to determine whether maintenance intravesical bacillus Calmette-Guerin (BCG) could reduce recurrence after transurethral resection of tumor 1 grade 3 (T1G3) superficial bladder cancer. All available published data of randomized clinical trials comparing transurethral resection plus intravesical BCG to either resection alone or resection plus another treatment on the treatment results in patients with superficial bladder cancer of T1G3 were selected for analysis. Both the fixed effects model and random effects model were applied, and the odds ratio () with its 95% confidence interval (CI) was used as the effect size estimate. Sensitivity analysis and publication bias determination were performed by funnel plots and comparing s of different models. Within the follow-up period, 375 of 915 (41.0%) BCG-treated patients and 332 of 733 (45.3%) non-BCG-treated patients developed tumor recurrence. In the combined results, a statistically significant difference in the s for tumor recurrence between the two treatment groups was found (randomized model combined effect 0.58, 95% CI 0.41 to 0.83, = 0.003). The stratified meta-analysis did not show any statistically significant confounding effects on the results when stratified by BCG strains. The randomized model combined effect of Pasteur F and other strains were 0.50 (95% CI 0.26 to 0.95, = 0.04) and 0.63 (95% CI 0.40 to 0.99, = 0.04), respectively. Therefore, we came to the conclusion that adjuvant maintenance instillation BCG combined with transurethral resection of bladder tumor (TURBT) is an effective conservative treatment for preventing recurrence of T1G3 bladder cancer.

Study of recombinant human IFN-α-2b bacilli Calmette–Guerin activated killer cells and against bladder

摘要: Presently, one of the most potent immunotherapies is the application of bacillus Calmette Guerin (BCG) to prevent recurrences of the superficial bladder cancer. Despite its successful use, nonresponders and certain side effects remain a major obstacle. Therefore, current studies aim at developing recombinant BCG (rBCG) strains secreting Th1-like cytokines to improve the effectiveness of the therapy. In this study, a new type of rBCG strain constructed by Tianjin institute of Urology was tested for its immunostimulatory capacity . Peripheral blood monocytes (PBMC) were stimulated by recombinant BCG and transformed into bacilli Calmette–Guerin activated killer (BAK) cells, and the effect of anticancer BAK cells was studied. Recombinant IFN--2b-BCG, wild-type BCG (wBCG), wild-type BCG and IFN--2b were coincubated with PBMCs respectively , and the proliferation of PBMC was detected with MTT in different time. BAK cells have the ability to kill bladder tumor cells, and the antitumor activity of effecter cells was determined by LDH release assay. The result of MTT showed that the proliferation of PBMC in the recombinant BCG group was more powerful than in the other two groups (<0.05). The result of LDH release assay showed that the antitumor activity of BAK cells stimulated by Recombinant BCG was the highest in all groups. We conclude that the recombinant BCG can activate more PBMCs to anti-bladder cancer than wild-type BCG does.

Expression and clinical implication of PRL-1 and PRL-3 in transitional cell carcinoma of bladder

摘要: The mRNA and protein expression of phosphatase of regenerating liver 1 (PRL-1) and phosphatase of regenerating liver 3 (PRL-3) in transitional cell carcinoma of bladder (BTCC) and normal epithelia of bladder was investigated, and the relationship between the BTCC and pathological changes was clarified. The expression of PRL-1 and PRL-3 mRNA was detected by using reverse transcription polymerase chain reaction (RT-PCR) in 30 cases of BTCC and 10 cases of normal bladder, and the expression of PRL-1 and PRL-3 protein was checked by using immunohistochemistry in 30 cases of BTCC and 15 cases of normal bladder. The expression levels of PRL-1 and PRL-3 mRNA and protein were higher in BTCC than those in normal bladder epithelia ( <0.05). The increased expression of PRL-1 and PRL-3 mRNA and protein was detectable in deep invasion and metastasis of BTCC ( <0.05). There was no correlation between the expression of PRL-1 and PRL-3 and gender, age or recurrence of BTCC (all >0.05). A significantly positive correlation was found between PRL-1 and PRL-3 in BTCC ( <0.05). PRL-1 and PRL-3 are expressed consistently and may contribute to the growth, differentiation, invasion and metastasis of BTCC.

近年来,人工智能在软件算法、硬件实现和应用等领域得到了迅速发展。本文综述了人工智能在生物医学中应用的最新进展,包括疾病诊断、生活辅助、生物医学信息处理和生物医学研究。综述的目的是跟踪新的科学成就,了解人工智能在生物医学中的巨大潜力和相关技术的适用性,并为相关领域的研究人员提供启示。可以断言,正如人工智能本身一样,人工智能在生物医学中的应用尚处于早期阶段。新的进展和突破将继续推进,不断深入并扩大广度,并在不久的将来迅速发展。本文以人工智能在癫痫发作和膀胱功能失调预测方面的应用的两个案例来说明其在生物医疗等方面的应用。
